Why you rank in your own suburb and vanish three suburbs over

A Miranda physio asked me a question I hear constantly in different costumes: "We come up first when we search from the clinic, but my patient in Engadine cannot find us at all. What is going on?"

What is going on is that the Google map pack is radius-based, and the Sutherland Shire's shape punishes businesses that do not plan for it. The Shire is long and thin, beaches in the east, bush in the west, and the map pack a searcher sees is drawn around them, not around you. From Engadine, Google sees closer options in Sutherland and Heathcote and serves those, even if you are better, cheaper and have triple the reviews. Proximity is the one ranking factor you cannot review your way out of.

What you can actually control

You cannot move your clinic, but three things genuinely shift how far your map presence reaches.

Review velocity and review text. The map pack rewards businesses that collect reviews steadily, not in bursts, and the words inside reviews act like keywords. A stream of reviews that mention "shoulder rehab" and "Engadine" literally extends the queries and suburbs you surface for. The businesses dominating the Shire map packs right now are not always the oldest, they are the ones that made reviews a habit, asking at the moment the customer is happiest, not in an email three weeks later.

Service-area pages that are real. A page for each key suburb works, but only when it is written with substance: driving directions from that suburb, jobs you commonly do there, the local detail that proves you actually service it. Ten thin pages that all say "we proudly service [suburb]" with the name swapped do more harm than good, and in 2026 both Google and its AI answers are aggressively ignoring them. Three genuine suburb pages beat ten templated ones.

Your categories and services list. Most profiles I audit in the Shire have one category set and the services section empty. Filling every applicable category and listing every service, with prices where you dare, is twenty minutes of work and it is often the difference between appearing for "physio" only and appearing for "sports physio", "clinical pilates" and "shoulder specialist" across the neighbouring suburbs too.

The honest limit

Even done perfectly, a Caringbah business will struggle to crack the map pack in Engadine, the geography is simply against you. That is where the suburb strategy from paid search takes over: a small, suburb-named ads campaign can buy the visibility on the far side of the Shire that the map will not give you organically. The two together, map strength at home and targeted ads where the map goes dark, is the whole local game plan for a Shire service business in a page: own your postcode organically, buy the next two.

If you want to know exactly which suburbs you are visible in today, I run a grid check that maps your ranking street by street across the Shire. It takes a day and ends the guessing. Ask me for one.
